Dental Plan
Our optional dental coverage is available to employees
and immediate family members. The plan covers preventative care, 80
percent minor restorative care, 50 percent major restorative care
and a portion of orthodontic care for dependents under age 23.

Medical Supply Discount
Floyd employees may order certain medical and some personal care supplies, including diapers and baby wipes from our Materials Management at a discounted price.

Life Insurance
Our insurance plan offers basic employee life insurance paid entirely by Floyd for full-time employees. The policy provides two times your annual salary, with a minimum amount of $35,000 coverage in the event of death. In addition, employees have the option to purchase supplemental and dependent life insurance.

Long-Term Disability
Floyd provides long-term disability coverage in the event that
you become disabled. After 90 days of missing work due to disability,
the plan will pay 60 percent of your annual base pay for up to five
years. Qualification for this benefit requires you are a full-time
employee for one year.

Medical Insurance
Floyd offers two medical insurance plans with four levels of coverage.
It is your choice which one best fits your lifestyle. Both plans
pay 90 percent of the prescription drug cost when purchased through
the Floyd Pharmacy.

Paid Time Off
Full-time employees receive two weeks vacation and seven holidays for the first five years of services. After five years, employees receive three weeks vacation plus seven holidays. Employees will accrue an extra five days per anniversary period. These days should be used for sick or unusual circumstances that are approved by a supervisor.
Part-time employees receive a pro-rated amount of paid time off that depends on their hours.

Retirement Plan
Floyd employees have the opportunity to invest in a 401k which is matched up to five percent. To be eligible the employee must be at least 21 years old, employed for 12 months and have 1,000+ working hours in a calendar year.

Spending Accounts
Floyd offers health care and dependent day care spending accounts. These allow employees to pay certain out-of-pocket expenses for health care and dependent day care on a tax-free basis.
Also, Employees can use a Health Care Spending Account for expected health care expenses that are not covered elsewhere and would be considered eligible deductions on your federal income tax return.

Vision Plan
The vision coverage is optional and is available to the employee and family members. The plan covers vision exam, glass lenses or contact lenses annually and a pair of frames biannually.
