Our commitment to excellence has led us to partner with Blue Ridge Area Health Education Center (AHEC) for the processing of all required student documentation. This documentation helps us to ensure that our patients are protected and our students are adequately prepared. 

Instructor Requirements

• Instructor Profile Form
  Instructors should compelete the basic Instruction Profile form before engaging in educational opportunities at Floyd.
  
  
• Instructor Approval Process
  Review and sign the Secuity and Confidentiality Statement. Fax to Blue Ridge AHEC at 866.521.1542 along with the your CV, background check, RN license number and proof of completion for the online orientation.
  
  
• Online Orientation
  Instructors must fulfill Floyd’s education requirement by completing the online non-employee orientation and submit a passing score.

Instructor Resources

• Affiliation Agreement
  Educational Institutions must have a current affiliation agreement with Floyd before their students can receive approval to do their clinical rotation. Schools can fax their agreements to Blue Ridge AHEC at

  Instructors are required to complete this form for all students who rotate at Floyd Medical Center.
  
  
• Immunization Requirements
  This form provides details about the immunizations required for all students.
